INTRODUCTION: Total knee arthroplasty (TKA) is a surgery that aims to restore function and relieve pain in advanced osteoarthritis. The Educational Workshop (EW) for TKA is given to patients in the pre-surgery period to inform them and facilitating their participation in their health process (empowerment). The aim of this study was developing and validating a self-administered questionnaire to evaluate the acquired knowledge after the EW by the patients who will undergo the TKA procedure. MATERIAL AND METHODS: It was a longitudinal and prospective observational study with a sample of TKA candidate patients. The phases for the construction and validation of this ad hoc questionnaire were: Phase 1: A panel of experts who agreed on a final questionnaire of 20 items; Phase 2: Pilot test administered to 47 patients; Phase 3: Final test of 11 items administered to 50 patients, before and after the EW; Phase 4: Re-test, after the EW and 2 weeks after, administered to 58 patients. RESULTS: One hundred and fifty five patients were included. The Cronbach's alpha coefficient for the item's internal consistency of the final questionnaire, 11 items, was 0.78. To reach the criterion validity, in pre-EW the mean number of hits was: 4.92 (SD=1.78) and in post-EW 10.68 (SD=0.55), a difference that had a statistical significance p<.0001, with no overlap in the 95% CI of the mean: 4.46-5.38/10.54-10.82. The test for stability and reliability, re-test, obtained a mean of right answers 10.87 (SD=0.33) and for the re-test of 10.70 (SD=0.59). The correlation of the interclass coefficient for the re-test was 0.99, which corresponds almost to a maximum concordance. CONCLUSIONS: The questionnaire developed in this study is a reliable and easy tool to evaluate the acquired knowledge in the EW for patients who will be TKA operated.

The study examines the use of macrobeads for the controlled-release of bacteria. Macrobeads were prepared by an easy dripping-technique using 20/80 wt/wt chitosan-starch blends and sodium tripolyphosphate as cross-linking agent. The resulting polymeric matrix was examined by SEM, XRD, TGA, and solid-RMN. The swelling-equilibrium, thermal behaviour, crystallinity, and size of macrobeads were affected by the autoclave-sterilization. The diameter of the sterilized xerogel was c.a. 1.6 mm. The results suggested that ionotropic-gelation and neutralization were the mechanisms underlying hydrogel formation. Plant growth-promoting bacteria (PGPB) were loaded into macrobeads separately or co-inoculated. Bacteria loaded macrobeads were dried and stored. Bacteria survived at least 12 months in orders of 109 CFU of A. brasilense/g and 108 CFU of P. fluorescens/g. Bacterial release in sterile saline solution tended to a super Case-II transport mechanism. Polymeric-matrix release efficiently both PGPB in natural soils, which uncovers their potential for the formulation of novel and improved biofertilizers.

Several apoplastic enzymes have been implicated in the control of elongation growth of plant cells. Among them, peroxidases contribute to both loosening and stiffening of the cell wall. They appear to be regulated by various mechanisms, including the action of extracellular inhibitors. To obtain evidence of the role of the enzyme-inhibitor interaction during leaf development, the intercellular washing fluids from Helianthus annuus leaves of different ages were isolated using standard methods of vacuum infiltration and centrifugation. Peroxidase activities, assessed using tetramethylbenzidine as substrate, increased during leaf development, reaching a maximum value after the leaves were fully expanded. An inhibitor, chemically characterised as ascorbate, co-localised with the enzyme in the apoplast. Moreover, there was a strong negative correlation between the action of peroxidase and the micromolar concentration of ascorbate in the apoplastic fluid. The results show that in growing leaves, the in planta ascorbate concentration is able to restrain peroxidase enzyme activity. Then, at the time of growth cessation, the loss of extracellular ascorbate relieves the inhibition on this enzyme that contributes to wall fixation.

Introducción: este estudio describe el dolor crónico como un estresor complejo al que la persona debe adaptarse continuamente usando diferentes estrategias. Método: el objetivo de esta investigación es observar las diferencias que existen en las variables psicológicas y el uso de las estrategias de afrontamiento del dolor en función del tiempo de evolución. Los instrumentos de evaluación son: "Cuestionario del Dolor McGill" (MPQ), "Estrategias de Afrontamiento del Dolor Crónico" (CADR), "Escala Hospitalaria de Ansiedad y Depresión" (HAD), "Escala de Catastrofismo del CSQ" (CSQ-C). Los 60 participantes del estudio son pacientes de una unidad de tratamiento especializada en el ámbito hospitalario. De estos se crearon dos grupos, uno de menor tiempo de evolución (de 1 a 5 años) y el otro de mayor tiempo de evolución (de más de 13 años). Resultados: los resultados muestran que tanto el tiempo de evolución como la edad del paciente se asocian con el uso de una estrategia pasiva, la "catarsis" y la evaluación del dolor tiene una relación positiva con el catastrofismo, las estrategias pasivas, la ansiedad y la depresión. Mediante el análisis de la comparación de medias de los dos grupos se confirma que el grupo de menor tiempo de evolución hace mayor uso de la estrategia de afrontamiento "catarsis" y además presenta puntuaciones más altas en "catastrofismo" en comparación con los componentes del otro grupo. Conclusión: se observa la relación entre del dolor y las variables psicológicas del estudio como también los cambios que pueden aparecer en diferentes etapas de la evolución del dolor


Introduction: preliminary study on the psychological variables and coping strategies in chronic pain related to duration. This study describes chronic pain as a stressor complex that the person using a coping strategies to adapt continuously. Method: the primary objective of this research observes differences in psychological variables and the use of pain coping strategies depending on the time of evolution. The assessment instruments are McGill Pain Questionnaire (MPQ), "Strategies for Coping with Chronic Pain" (CADR), Hospital Anxiety and Depression Scale (HAD), Catastro-phism Scale CSQ (CSQ-C). Participants of this study are patients with chronic pain and are treated in to specialized unit. The results show that duration and patient's age are associated with the use of a passive strategy, the "catharsis". The perception of pain has a positive relationship with catastrophizing, passive strategies, anxiety and depression. Results: results obtained by comparing the average of the two groups, one with 1 to 5 years of evolution, and another 13 to 33 years, show that the group of shorter duration makes greater use of the coping strategy of catharsis and it has scored higher on catastrophizing compared to another group. Conclusion: we observe the relationship between pain and psychological variables of this study and changes that may occur in different stages of duration of pain (AU)

Objetivo. Demostrar las alteraciones neurofisiológicas y del comportamiento del ciclo vigilia-sueño características del insomnio familiar fatal (IFF), en un paciente con sospecha clínica y posterior diagnóstico genético. Paciente y método. Varón de 39 años. Presenta historia familiar compatible con IFF. En el curso de 5 meses manifiesta impotencia, insomnio progresivo y episodios diurnos de sueño con apneas, mioclonías, automatismos y gesticulaciones. Al ingreso, presenta diplopía, hipertensión arterial e hiperhidrosis. Se realizó una monitorización videopolisomnográfica durante 24 h. Cada segundo de registro fue clasificado según los criterios de Sforza (1995). Resultados. Vigilia: ojos cerrados, actividad motora en miembros inferiores. Ritmo alfa posterior reactivo a 8-9 Hz. Sueño: breves episodios de sueño no REM y REM, de características atípicas, con ausencia de sueño profundo y disminución de actividad spindle, en asociación con mioclonías, gesticulaciones y apneas. Insomnio de conciliación. Tras indicar alprazolam (1 mg por vía oral y 0,5 mg sublingual), presentó patrón no REM durante 3,5 h, sin actividad motora y sin apneas. Discusión y conclusiones. Los hallazgos polisomnográficos se caracterizaron por grave alteración de la organización cíclica del sueño, disminución del tiempo total de sueño y patrones no REM y REM atípicos. La administración de alprazolam permitió aumentar el sueño nocturno, con desaparición de las sacudidas motoras y las apneas (AU)


Objective. To demonstrate the neurophysiologic and behavior disorders in the wake-sleep cycle typical of fatal familial insomnia (IFF), in a case with clinical suspect and latter genetic diagnostic. Patient and method. Male 39 year old. Family history of an IFF syndrome. Within 5 months he developed impotence, progressive insomnia and episodes of daytime somnolence associated with apneas, myoclonus and anormal motor behavior. He was hospitalized with diplopia, high blood pressure and hyperhidrosis. We carried out a video-polisomnografic long-term monitoring during 24 hours. Each second of the record was classified according to the de Sforza criterium (1995). Results. Waking: closed eyes, restless movements of legs. Responsive posterior alpha rhythm at 8-9 Hz. Sleep: brief episodes of "NREM" and "REM" sleep of atipical features, such as absence of slow wave sleep and marked reduction of spindle frequency activity, with myoclonus, gesturing and apneas. At night, impossibility in falling asleep. After alprazolam, "NREM" sleep during 3,5 hours, without motor activity nor apneas. Discussion and conclusions. The polisomnografic findings characterized by severe alteration of the cyclic sleep organization, reduction in total sleep time and atipical patterns of "NREM" and "REM" sleep. Administration of alprazolam (1 mg orally and 0.5 mg sublingually) allowed increasing night-time sleep, dissapearing jerks and apneas (AU)
